Jose had been struggling with diabetes for years, trying to find ways to control his glucose levels. He had been to many different doctors and clinics, but none of them had ever made him feel heard or truly cared about his health problems. That all changed when he was referred to North Texas Area Community Health Centers (NTACHC) by the Refugee and Immigrant Center for Education and Legal Services (RAICES).

At his first appointment, Jose was amazed. Not only did he feel welcomed and heard, but the cost of care was far more affordable than most of the other options he had tried in the past. He was also referred to the Living Your Best Life (LYBL) program, which taught him how to make healthy changes to his lifestyle and diet that would help him lower his glucose levels. LYBL is an NTACHC program in which providers refer patients needing counseling to help get their diabetes and high blood pressure under control.

Jose was also able to take advantage of new technologies that helped him monitor his blood pressure remotely. The remote blood pressure monitoring cuff allowed Jose to take his reading and instantly transmit the data to a secure portal where his healthcare providers were able to review it in real-time. But even more important was the care he received from the staff at NTACHC. Gaby, Carmen, and Mary Lou were instrumental in helping him make the changes that would improve his health.

Carmen said, “A patient’s overall wellness is our priority. We wanted to make sure Jose truly felt that we care for him and take his health seriously.”

Thanks to the help of the staff at NTACHC and the Living Your Best Life program, Jose was able to lower his glucose levels and feel better than ever before. He was so impressed with his care that he recommended NTACHC to several of his friends. He was confident that anyone who sought care at NTACHC would be in good hands.

“This has helped me a lot,” Jose said. “I tell everyone I know about NTACHC and the amazing care they provide, and the affordable cost of medications.”

Jose was grateful he had been referred to NTACHC and was glad he finally found a center that was willing to take care of him and make sure he was living his best life.

Fort Worth, Texas, August 29, 2023 — North Texas Area Community Health Centers (NTACHC) have achieved the significant milestone of accreditation by the Association of Diabetes Care & Education Specialists (ADCES). Accreditation represents a high level of quality and service to the community, and the ability to better meet the needs of Medicare recipients and others affected by diabetes in the Tarrant County area.

“We’re honored that we’ve received this prestigious accreditation as we provide high quality diabetes management through our Living Your Best Life (LYBL) program that helps our patients manage their diabetes and hypertension.

Current NTACHC patients are referred by their providers to join the Living Your Best Life Program where they are taught how to control their diabetes and hypertension through proper meal planning, exercise and healthy habits such as quitting smoking. NTACHC has a few staff members who work with patients one-on-one to provide this service in both English and in Spanish.

Diabetes education services utilize a collaborative process through which people with or at risk for diabetes work with a diabetes educator to receive individualized care to help them reach their health goals. Diabetes education is a covered Medicare benefit when delivered through an accredited program.

“Evidence shows diabetes education and support is helping people with diabetes to lower their A1c’s and obtain the knowledge and skills to better self-manage their condition,” said Sacha Uelmen, Director of Accreditation at ADCES. “That is why we need programs like [Program name] to make these services available in their community.”

ADCES is a National Accredited Organization (NAO) for the Centers for Medicare and Medicaid Services (CMS). Accredited programs must meet the 2022 National Standards for Diabetes Self-Management Education and Support; 6 standards that are revised every 5 years by a group of diabetes educators representing both ADCES and the American Diabetes Association and approved by CMS. Programs must maintain compliance with the Standards during the 4-year accreditation cycle.

More than a year ago, Guadalupe noticed that she wasn’t feeling like her best self and was experiencing high volumes of stress. She knew that she wanted to take control of her well-being and decided to reach out to North Texas Area Community Health Centers (NTACHC). Guadalupe was drawn to NTACHC because they were genuinely interested in her health and offered assistance for her financial situation.

During her initial visit, Guadalupe learned that she was struggling with uncontrolled diabetes. Her provider suggested taking insulin, but she was hesitant. After discussing different options with her provider, Guadalupe agreed that if she made lifestyle changes over the next few months, she may not have to take the medication. As an effort to help Guadalupe with the changes, her provider referred her to NTACHC’s Living Your Best Life Program (LYBL) – a program that’s designed to help individuals control their diabetes and high blood pressure. NTACHC has assigned staff members who are nurses who help people in this important program.

While participating in LYBL, Guadalupe was taught healthy cooking and eating habits in a group setting. She was also provided with educational materials to help her set realistic goals and learn how to address barriers that may keep her from being as healthy as possible. Shortly after, Guadalupe was connected to Rosa es Rojo. Rosa es Rojo is an NTACHC partner that provides education on nutrition, emotional health, positive thinking and physical activities for Hispanic women in North Texas.

Thanks to LYBL and Rosa es Rojo, Guadalupe’s life has been tremendously impacted. Not only was she able to reverse her diabetes, but she lost weight. She remarked, “Finding NTACHC was a huge blessing.” Anytime Guadalupe comes across someone who has a similar problem, she is quick to recommend them to NATCHC. “My life has changed, and it’s visible. NTACHC has shown me a new way of living, and I don’t want to go back to my unhealthy ways.”

Ximena gets fitted for glasses to see clearly at Northside Eye Care Center in Fort Worth.

A special thanks to Communities Foundation of Texas for awarding North Texas Area Community Health Centers a grant from their recent health-focused funding cycle to provide Northside Eye Care Center with pediatric eye care services. Here’s a story of how this grant is helping one of our youngest patients.

Helping people with their eyesight is a passion of Dr. Mary Kate Walters, director of Northside Eye Care Center and professor of practice with University of Houston College of Optometry. She and her team are committed to helping people see clearly.

Dr. Walters met Ximena, a precious five-year-old who had trouble seeing. During this past academic year, Ximena’s mom, Keyla, received a letter from her daughter’s school stating that she had recently failed an eye exam. Though Ximena was provided with corrective lenses at the time of the exam, Keyla was still concerned.

Keyla quickly contacted her insurance provider to schedule a more comprehensive exam as soon as possible at a nearby facility. Keyla was referred to Northside’s Eye Care Center, which is conveniently located close to her home. The facility also accepted her insurance, which had been an issue with a few other places. She was surprised to find that the Northside Eye Care Center offered an array of services, ranging from routine eye care to comprehensive medical exams, for patients of all ages using state-of-the-art equipment and technology.

At the appointment, Ximena received a thorough examination and was provided with a more appropriate lens prescription. They were even able to easily switch out the lenses on Ximena’s original pair of frames. Keyla was relieved that her daughter got the help she needed and is glad to know that Ximena will be able to see more clearly. Keyla stated that working the team “has been great. Ximena loves it there!” Keyla also recommends Northside Eye Care Center as there’s a “great group of doctors” ready to help.

Northside Eye Care Center is located on the second floor of North Texas Area Community Health Centers (NTACHC) located at 2332 Beverly Drive in Fort Worth. Northside Eye Care Center’s team is accepting new patients and provides pediatric eye exams, diabetic eye exams and treatment for glaucoma, cataracts and red eyes. The place partners with Lighthouse for the Blind to provide glasses and fittings at its location for a one-stop shop. The Northside Eye Care Center (located at 2332 Beverly Hills Drive in Fort Worth) helps patients like Daniel and Angelina with routine eye care as well as treatment for serious vision problems. Over the last few years, Daniel and his wife, Angelina, both found themselves facing increasingly serious problems with their vision.

Daniel had a large cataract and proliferative diabetic retinopathy, an advanced stage of diabetic eye disease and a leading cause of blindness in adults. Both conditions urgently required surgery. Angelina was struggling with diabetic retinopathy as well.

The couple had health insurance, but the only in-network surgeons on their plan were an hour or more away, which posed transportation issues. Desperate for a solution, and despairing that one even existed, Daniel and Angelina turned to North Texas Area Community Health Centers (NTACHC’s) Northside Eye Care Center for help. From this point forward, a brighter future began to come into focus for the couple, quite literally. 

“I was losing sight in my right eye, and also seeing black lines,” recalls Angelina. “I was very scared about what might happen next, and depressed about how fast my vision was decreasing.”

Daniel was equally worried about his eyesight. “I had cataract surgery in one eye four years ago, but the health center was too far away, and the surgery was very expensive,” he says. “The health center never bothered to call afterwards and help me figure out the surgery needed for my other eye, so I didn’t proceed.”

Both Daniel and Angelina were patients at NTACHC, so they asked their primary care doctors for a referral to the recently-opened Northside Eye Care Center, located on the second floor of the Northside location offering eye care for patients of all ages, including care related to glaucoma, diabetes, macular degeneration, cataracts, dry/red eye, injury/infection and prescriptions.

“The first step was working with the family to help them switch to a different insurance plan that would better position them to obtain care,” says Dr. Mary Kate Sain, Clinic Director of the Northside Eye Care Center.  “Their new plan had an in-network cataract surgeon in the area, but no retinal surgeons within 100 miles. It was a long road, but eventually we found a surgeon in this area who worked with the Daniel and Angelina to successfully file a *gap exception request with their insurance provider, which enabled them to access affordable care and reduce the risk of irreversible vision loss.” (*A gap exception allows policyholders to see out-of-network providers at in-network costs.)

Today, Daniel and Angelina are back on the road to better health and better vision, an outcome the pair attributes to Northside Eye Care Center’s Dr. Sain who has coordinated their overall care. 

“My eyesight has improved so much,” says Angelina. “Our experience at Northside Eye Care Center has been exceptional, like nothing I’ve ever had in my life. We’ve had so many problems in the past, but Dr. Sain and her staff always work with us to find a solution. I feel so cared for here that I actually look forward to my upcoming appointments.” 

Daniel adds, “I am now able to enjoy my life with my loved ones, and for that I give thanks to Dr. Sain. She is very thorough with all of her patients. If you are having any type of eye problems, come to the Northside Eye Care Center.  They will take care of you 100 percent!”
